What the columns mean
Status is quoted from the registry. “Active, not recruiting” means the trial is running but has stopped enrolling — participants are still being followed. “Completed” means follow-up has finished, not that results have been published.
Enrolment is marked actual or estimated because the difference matters. An estimated figure is a target; an actual figure is what happened. Two trials show a small discrepancy between the registry count and the number Lilly stated in its press release — 2,335 against 2,339 for TRIUMPH-1, and 1,946 against 1,949 for TRIUMPH-3. Neither is large enough to change anything, and neither is worth hiding.
Results distinguishes topline from peer reviewed. Topline means the sponsor issued a press release. Peer reviewed means the full dataset went through external review and was published. Four of the five disclosed readouts are topline only.
The three programmes
TRIUMPH is the obesity programme — TRIUMPH-1 through TRIUMPH-9 plus TRIUMPH-Outcomes, covering obesity, obesity with type 2 diabetes, severe obesity with cardiovascular disease, knee osteoarthritis, chronic low back pain, weight maintenance and dose-escalation schemes.
TRANSCEND-T2D is the type 2 diabetes programme, including head-to-head comparisons against semaglutide and studies in renal impairment.
SYNERGY-Outcomes is a master protocol in metabolic dysfunction-associated steatotic liver disease testing several agents; retatrutide is one arm, not the whole trial. It does not complete until 2032.
Two trials are worth singling out because they answer questions the others cannot.TRIUMPH-Outcomes is the cardiovascular and kidney outcomes study — 10,000 participants, event-driven, not expected to complete before 2029. It is the trial that would establish whether retatrutide prevents heart attacks and strokes, as opposed to improving the numbers associated with them.TRIUMPH-5 compares retatrutide directly against tirzepatide, which is the only way to answer whether it is better than what already exists.
A registry entry we are not counting
ClinicalTrials.gov contains one further retatrutide record, NCT07467447, registered by a sponsor other than Eli Lilly. Registered by a sponsor other than Eli Lilly, but reusing Lilly's official study title and Lilly's internal protocol identifier from a completed Phase 2 trial. The relationship between that sponsor and Lilly could not be verified from any primary source, so this record is not presented as part of Lilly's retatrutide programme. It is noted here rather than silently dropped, but it is not presented as part of Lilly's programme.
Earlier phases
Beyond Phase 3, the registry holds four Phase 2 studies and fifteen Phase 1 studies. Two Phase 2 trials — in obesity and in type 2 diabetes — have posted results on the registry and were published in 2023 in the New England Journal of Medicine and The Lancet respectively. Those are the trials that made the Phase 3 programme happen.
