How much will retatrutide cost?

Nobody knows, including Lilly. Here is what determines the answer, and why the numbers circulating now are not it.

No official U.S. list price has been announced. Retatrutide is investigational and is not sold as a medicine, so it does not have a price. A list price is set around approval and launch, neither of which has happened.

Why there is no price

A drug's list price is set by its manufacturer at launch. Launch follows approval. Approval follows a review the FDA has not begun, of an application that has not been filed. There is no mechanism by which a price could exist right now, and no reason for Lilly to have decided one internally, let alone published it.

This site will not forecast a number. Estimates built from what other incretin drugs cost are guesses dressed as analysis, and repeating them would give a fabricated figure the appearance of a fact simply by putting it next to real ones.

What will determine the price, if it is approved

These are the factors that decide, and none of them is settled.

  • The approved indications. A drug approved for obesity, knee osteoarthritis pain and sleep apnea is positioned differently from one approved only for weight management. Which indications any label would carry is unknown.
  • The competitive field at launch. Tirzepatide and semaglutide are already established, and the pricing of incretin drugs has moved considerably in response to competition and direct-to-consumer programmes. What that field looks like in 2027 or 2028 is not predictable from here.
  • Outcomes evidence. Payers pay more for drugs shown to prevent expensive events. TRIUMPH-Outcomes, the cardiovascular and kidney outcomes trial, is not expected to complete until 2029 — after any plausible launch. Its result would affect coverage more than almost anything else.
  • Rebates and net price. The list price of an incretin drug and what anyone actually pays have diverged sharply. A list price, when it comes, will not tell you the cost to a patient.
  • Coverage policy. Commercial insurers, Medicare and Medicaid make separate decisions with separate rules, and coverage for obesity medicines specifically has been contested.
